MINOR MEDICAL CARE
I give permission for Common Threads Farm staff to apply sunscreen and administer minor medical care as needed.
EMERGENCY MEDICAL CARE
I hereby give my consent for my child(ren), as listed above, to be given emergency treatment by a qualified staff member at Common Threads Farm, or transported to the hospital for treatment if deemed necessary. In the event that I cannot be contacted, I further consent to the medical, surgical and hospital care, treatment, and procedures to be performed for my child by a licensed physician or hospital when deemed immediately necessary or advisable by the physician to safeguard my child(ren)'s health.
I hereby acknowledge that I have voluntarily chosen to allow my child(ren) to participate in the Common Threads Farm Program located at Western Washington University’s The Outback Farm (hereinafter called “Program”). I have consulted with my child(ren)’s health-care provider(s), to the extent I believe necessary, and I have determined that my child(ren) is(are) both physically and mentally fit to participate in the Program. I understand the Program includes risks that are present in everyday gardening and farming activities. I acknowledge that my child(ren) will be learning and working in an outdoor environment and exposed to: unpredictable weather; slips, trips and falls; using tools and equipment; farm and wild animals; insects; water hazards; physical exertion; organic farm chemicals; and the possible reckless or negligent behavior of other individuals. I agree to accept those risks and others that my child(ren) may experience. I also agree that Western Washington University and Common Threads Farm are not responsible or liable for any injury or illness to my child(ren), unless such injury or illness is caused by their negligence or willful misconduct, or that of their officers, agents, volunteers, or employees.
